Medical Hill Healthcare is seeking a compassionate Registered Nurse (RN) Case Manager to coordinate comprehensive, resident-centered care and ensure effective communication among residents, families, physicians, nursing staff, and interdisciplinary teams.

The ideal candidate holds a California RN license and has experience in case management, care coordination, or skilled nursing, with strong assessment and communication skills and a dedication to improving resident outcomes.

Qualifications

• California RN license required.
• Experience in case management, care coordination, or skilled nursing preferred.

• Ability to work effectively with residents, families, and interdisciplinary teams.

Responsibilities

• Coordinate and oversee individualized care plans for residents.
• Monitor resident progress and identify changes in condition or care needs.
• Collaborate with physicians, nursing staff, therapy, social services, and other team members.
• Conduct assessments and assist with care plan development and updates.
• Facilitate communication with residents and families regarding care needs and services.
• Monitor hospitalizations, transfers, discharges, and transitions of care.
• Identify barriers to care and develop interventions.
• Maintain accurate, timely, and compliant clinical documentation.
• Participate in care conferences and interdisciplinary meetings.
• Support quality improvement and regulatory compliance initiatives.
• Promote continuity of care and advocate for resident needs.
• Perform other duties as assigned.
