Job summary

Medical Hill Healthcare is seeking a compassionate Registered Nurse (RN) Case Manager to join our care management team. This role coordinates comprehensive, resident-centered care and ensures effective communication among residents, families, physicians, nursing staff, and the interdisciplinary team.

The ideal candidate is an experienced RN with California licensure, strong clinical judgment, and excellent communication and organizational skills.

Qualifications

  • Current and active California RN license.
  • Experience in case management, care coordination, or skilled nursing preferred.
  • Strong clinical assessment and critical-thinking skills.
  • Excellent written and verbal communication skills.
  • Strong organizational and time-management abilities.
  • Ability to work effectively with residents, families, and interdisciplinary teams.
  • Knowledge of care planning, documentation, and regulatory requirements.
  • Professional, dependable, and compassionate approach to resident care.

Responsibilities

  • Coordinate and oversee individualized care plans for residents.
  • Monitor resident progress and identify changes in condition or care needs.
  • Collaborate with physicians, nursing staff, therapy, social services, and other members of the interdisciplinary team.
  • Conduct assessments and assist with developing and updating care plans.
  • Facilitate communication with residents and families regarding care needs and services.
  • Monitor hospitalizations, transfers, discharges, and transitions of care.
  • Assist with identifying barriers to care and developing appropriate interventions.
  • Maintain accurate, timely, and compliant clinical documentation.
  • Participate in care conferences and interdisciplinary meetings.
  • Support quality improvement and regulatory compliance initiatives.
  • Promote continuity of care and advocate for resident needs.
  • Perform other duties as assigned.

Salary: $105,000-$115,000 per year

Medical Hill Healthcare is seeking a compassionate, organized, and experienced Registered Nurse (RN) Case Manager to join our team. This position is responsible for coordinating comprehensive, resident-centered care and ensuring effective communication among residents, families, physicians, nursing staff, and interdisciplinary team members.
