• Minimum of one year of RN experience in acute care; home care experience preferred.

Responsibilities

• Oversees patient care from start to end of the episode.
• Updates doctor orders and notifies changes as needed.
• Provides skilled nursing care per physician orders in the patient's home.
• Performs OASIS assessments and develops a care plan.
• Documents care and maintains compliance with Medicare guidelines.
• Collaborates with LPNs and aides; supervises team.
