Job summary

Summa Health in Akron, OH is seeking an experienced nursing leader to develop, implement, coordinate, and evaluate complex care plans for at-risk patient populations. Working with the interdisciplinary team, you’ll guide patients through the system and coordinate care among physicians, staff, and external facilities.

You will assess patient needs, lead care plan implementation, and ensure delivery of high-quality, patient-centered care across Summa's network.

Qualifications

• BSN degree and Ohio license to practice
• CCNE- or ACEN-accredited nursing program
• 5 years in nursing with leadership/management experience

Summary

Develops, implements, coordinates and evaluates complex care plans targeting at-risk patient populations in consultation with the interdisciplinary (ID) team, which includes the patient and family. Directs the patient through the system by coordinating care needs among the physicians, direct care practice staff, interdisciplinary team and outside facilities/agencies. Comprehensively assesses the patient’s needs – and helps implement recommendations made by the team.

Formal Education Required

• Bachelor’s of Science in Nursing (BSN)
• Graduate of a program of professional nursing that is accredited by the Commission on Collegiate Nursing Education (CCNE) or Accreditation Commission for Education in Nursing (ACEN)
• Current license to practice registered nursing in the State of Ohio

Experience & Training Required

• Five (5) years in nursing care relevant to patient population to include leadership/management experience with demonstrated success
