Job summary

AMN Healthcare seeks an RN Case Manager in Lewes, Delaware to provide comprehensive case management for medically complex inpatients. You will perform nursing assessment, create and monitor care plans, and coach patients and families toward self-management and adherence to the care plan.

The role requires a nursing degree (BSN preferred) or ADN with extensive case management experience, with Epic experience preferred. Work hours are 8a-5p, Monday to Friday, with weekend rotation.

Qualifications

• Bachelor's degree in nursing or related field OR ADN with 5 years of case management experience.
• BLS is not required.
• Epic experience preferred.

Responsibilities

• Provide case management services for medically complex inpatients.
• Develop patient and family care plans and self-care coaching.
• Coordinate care with interdisciplinary team and ensure culturally sensitive care.
