Job summary

Covenant HealthCare is seeking an ECC Case Management Specialist (RN) to join a multidisciplinary care team. The role focuses on promoting optimal clinical and resource outcomes, managing length of stay, and functioning as the key information resource for the team.

The specialist will work across patient populations, collaborating with physicians and families to ensure safe discharge planning and efficient care transitions, with emphasis on geriatric care and quality improvement.

Qualifications

• RN with a bachelor's degree or pursuing within 6 months with program completion within 3 years; minimum 3 years acute care experience.
• RN with ADN and state license, 2 years bedside nursing, 5 years in acute care and 5 years in case management.

Responsibilities

• Facilitate collaborative management of patient care across the continuum and remove barriers to timely care delivery.
• Assist patients and families to cope with hospitalization and help with tangible needs affecting discharge planning.
• Coordinate transfers to appropriate care levels and ensure CMS discharge planning compliance.
• Participate in process improvement and documentic outcomes, ensuring high-quality, cost-effective care.
