Job summary

Heart to Heart Hospice is seeking an experienced RN Case Manager to assess and coordinate comprehensive home-based patient care in the Dallas area. The role requires an active RN license, reliable transportation, and 2+ years of clinical nursing experience.

You will develop and implement care plans, collaborate with IDG members, and ensure timely documentation and regulatory compliance. This full-time position offers competitive pay and benefits in a supportive team dedicated to patient-centered

Qualifications

Graduate of an accredited Diploma, ADN, or BSN program.
Current State RN license.
Driver's license with reliable transportation and valid auto liability insurance.
Two years of RN experience preferred in healthcare/clinical setting.

Responsibilities

Initial and ongoing comprehensive assessments of the impact of the terminal disease on patient needs and ADLs.
Implement the individualized POC and revise as necessary.
Collaborates care among patient, caregiver, physicians and healthcare professionals.
Attend IDG meetings and coordinate patient care planning with the IDG.
Perform on-call responsibilities and provide on-call service to patients/families.
Timely submission of all documentation
Performance of other duties as required
Ensure compliance with laws, Medicare regulations and policies.
