Job summary

Heart to Heart Hospice is seeking an RN Case Manager to perform comprehensive assessments of patients with life-limiting illnesses and to develop and implement individualized care plans. The role involves coordinating with physicians and the care team to ensure holistic, compliant patient care and timely documentation.

The position requires an active RN license, a valid driver's license, and access to reliable transportation for field visits.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State Registered Nurse License
• Current Driver's License with reliable transportation and ongoing auto liability insurance
• Two years RN experience preferred in healthcare/clinical setting

Responsibilities

• Conduct initial and ongoing assessments of the terminal illness impact on patient physical, psychosocial, and environmental needs and ADLs
• Implement and revise the patient care plan (POC) as needed
• Coordinate care among patient, caregiver, physicians and healthcare professionals
• Attend IDG meetings and participate in patient care planning and prioritization of needs
• Provide on-call coverage and on-call service to patients/families as assigned
• Ensure timely submission of all documentation
• Comply with local, state and federal laws and Medicare regulations
