Community Care is hiring a RN Case Manager to join our Family Care Program in the Waukesha location. This full-time role focuses on team-based community case management to help elderly and disabled adults live independently in the community.

You will conduct home visits, develop care plans, and coordinate services with families and supports. A WI RN license and eligibility for LTC screener certification are required, with a signing bonus offered.

Competenze

Education: Associates degree in Nursing required; Bachelor's degree preferred.
Wisconsin Licensed Registered Nurse.
Eligibility for long-term care functional screener certification is required.
WI Driver's License.
2 years of recent experience with older adults or persons with physical or intellectual disabilities in a nursing capacity required; community nursing preferred.

Mansioni

Completes discipline-specific initial and ongoing assessments; conducts home visits and consults with families and other supports for additional information.
Develops and maintains care plans, including long-term care and personal outcomes; ensures quality, coordination and effectiveness of services.
