Job summary

Full Circle Health in Boise, ID is seeking a Clinical Case Manager to join the Raymond Clinic team. The role focuses on coordinating care for patients with chronic or complex conditions and requires close collaboration with clinicians, patients and community partners.

Requirements include an Idaho RN or LPN license or a Master’s in Social Work, current BCLS, and CCM certification preferred. Two years of clinical practice is required; pediatrics experience is a plus but not required.

Qualifications

• Valid Idaho RN or LPN license; Master’s prepared Social Worker also acceptable.
• Current BCLS certification.
• Certified Case Manager certification preferred.

Responsibilities

• Coordinate care for individuals with chronic or complex conditions.
• Identify, screen, assess, plan, implement and monitor healthcare services.
• Collaborate with providers and interdisciplinary team to optimize patient outcomes.
