Job summary

Nova Healthcare Administrators, Inc. is seeking a Case Manager (RN) in a remote role based in Buffalo, NY. The incumbent will apply core case management activities with focus on assessment, planning, implementation, coordination, monitoring and evaluation.

Requirements include an active RN license in an NLC state and CCM certification (or within 3 years of start). Three years RN experience in med/surg or ambulatory settings is required, with managed care experience a plus.

Qualifications

• RN with active, unrestricted license in an NLC state.
• CCM certification required; must be obtained within 3 years if not yet certified.
• 3 years of clinical RN experience in med/surg or ambulatory care.
• Managed care experience is a plus.
• Ability to interact with physicians and other health‑care team members.
• Strong communication, time management and problem‑solving skills.

Responsibilities

• Assess the patient’s needs using medical and social histories.
• Develop a care plan considering assessments, benefits, and cost impact.
• Continuously evaluate and adjust the plan within case management scope.
• Ensure regulatory privacy compliance and coordinate documentation.
• Build relationships with the interdisciplinary team and communicate objectives.
• Advocate for patients and connect them with community resources.
• Identify cases needing alternative care levels and coordinate with payors.
• Document plans in a timely manner and participate in ongoing training.
