Qualifications

Responsibilities

• Develop and continuously evaluate patient-centered plans of care based on clinical, psychosocial, and cost factors.
• Coordinate with payors, providers, and interdisciplinary team members to ensure quality and cost-effective services.
• Maintain compliance with regulatory and privacy standards, and document plans of care timely.
