Job summary

HCA Houston Healthcare Clear Lake seeks a Registered Nurse Case Manager (PRN) to support patients and families through the care journey. You will coordinate services, connect resources, and develop care plans with physicians, nurses, and department leaders to ensure safe transitions and consistent quality across care continuum.

Qualifications

• RN Case Manager experience and licensure required.
• Associate Degree required to practice as an RN.
• 2 years Case Management experience preferred.
• 3 years RN experience in an acute care setting required.

Responsibilities

• Guide patients and families through program orientation, discharge criteria, and patient rights.
• Coordinate education for patients, families, and caregivers in treatment planning.
• Develop and implement individualized treatment plans reflecting patient needs and goals.
• Complete psychosocial assessments and discharge planning evaluations, documenting findings clearly.
• Collaborate with Rehab Program Director and care team to ensure seamless, patient-centered care.
• Promote HCA Healthcare values through every patient and family interaction.
