Job summary

HCA Houston Healthcare Southeast in Pasadena, TX is seeking a PRN RN Case Manager to support patients and families through the care journey. You will coordinate services, develop care plans, and ensure safe transitions in collaboration with physicians, nurses, and leaders across award-winning specialty services.

Qualifications

• Graduate of an accredited diploma, associate, or baccalaureate nursing program.
• Bachelor’s Degree in Nursing preferred.
• Current Registered Nurse License in the State of Texas or Multi-State Compact License (RN Compact License must obtain Texas RN License within 90 days of hire).
• 2+ years in case management OR 3+ years in clinical nursing.

Responsibilities

• Guide patients and families through program orientation, explaining rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights.
• Coordinate education for patients, families, and caregivers for participation in treatment planning and family conferences.
• Develop and implement individualized treatment plans reflecting patient strengths and needs.
• Complete psychosocial assessments and discharge planning evaluations, documenting findings and communicating needs across the care team.
• Collaborate with Rehab Program Director, Facility Case Management Director, and interdisciplinary team for seamless, patient-centered care.
• Promote HCA Healthcare’s values through every patient and family interaction.
