Job summary

MercyOne North Iowa Medical Center is seeking a Care Transitions Case Manager. This role focuses on coordinating patient care progression across the continuum, advocating for optimal outcomes and ensuring timely, appropriate care levels.

The position requires an Iowa RN license, a BSN or approved BA (or willingness to obtain within three years), plus five years of clinical RN experience. Case management certification is preferred.

• Current and valid RN license to practice in Iowa.
• BSN or approved BA is required or willing to obtain within three years.
• Five years of clinical RN experience.
• Case management certification preferred.
• Diverse knowledge of community services, resources and programs.

Responsibilities

• Coordinates and facilitates patient care progression across the continuum.
• Collaborates with physicians, social services and nurses to optimize outcomes.
• Maintains timely discharge planning with patients, families and providers.
• Identifies and resolves barriers to discharge and transitions of care.
• Documents discharge plans and participates in interdisciplinary rounds.
• Promotes confidentiality and adheres to HIPAA and organizational policies.
• Supports post-acute referrals and coordinates with care teams at next site.
