Job summary

MercyOne in Mason City, IA seeks a Care Transitions Case Manager to coordinate patient care progression and ensure seamless transitions across care settings. You will collaborate with physicians, social services, nurses, and community partners to plan cost-effective, high-quality care while supporting timely discharges.

The role requires an Iowa RN license with BSN/BA and at least five years of clinical RN experience; case management experience and CM certification are preferred.

Qualifications

  • Current and valid Iowa RN license; BSN or approved BA is required or willing to obtain within three years of employment
  • Five years clinical RN experience required
  • Diverse knowledge of community services, resources and programs
  • Previous case management experience preferred
  • Case Management Certification preferred

Responsibilities

  • Coordinates/facilitates patient care progression throughout the continuum while advocating for the patient’s best possible outcome
  • Works with interdisciplinary rounds to ensure timely, appropriate patient management
  • Coordinates with social workers to ensure smooth handoff at next site of care
  • Identifies and resolves delays to discharge; collaborates with patients, families, physicians, payors, and providers
  • Conducts face-to-face assessments and documents discharge plans for all patients
  • Serves as resource for families, patients, employees, and other health care professionals
  • Assists in creating processes to address post-acute referrals, denials, readmissions, and length of stay
  • Applies quality improvement methods to improve case management processes at MercyOne North Iowa Medical Center
  • Participates in team meetings regarding patient needs affecting cost, quality, and length of stay

The Care Transitions Case Manager collaborates with physicians, social services, nurses, community agencies and other members of the care team to define care options and resources, to plan cost-effective quality care and to achieve optimal quality outcomes. The Care Transitions Case Manager is primarily responsible for selection of and ensuring the timely and seamless clinical transition of patients through medically appropriate levels of care.

Schedule:

Monday - Friday

6:30 am - 3:00 pm

8 hour shifts

Rotating weekends and holidays

Qualifications:

  • Current and valid license to practice as Registered Nurses in Iowa, BSN or approved BA is required or willing to obtain within three years of employment

  • Case Management Certification preferred

  • Five years Clinical RN experience required

  • Diverse knowledge of community services, resources and programs

  • Previous case management experience preferred

What you’ll do:

  • Coordinates/facilitates patient care progression throughout the continuum while advocating for the patients best possible outcome

  • Works collaboratively and maintains active communication through interdisciplinary rounds with physicians, nursing and other members of the multidisciplinary team to effect timely, appropriate patient management

  • Coordinates with social workers to ensure a warm handoff with caregivers at next site of care including but not limited to health coaches, home care agencies, and nursing homes or skilled facilities

  • Proactively identifies, triages, and assists in resolving delays and obstacles to discharge. Responsible for assuring through case management assessment, as well as early and ongoing discharge planning by collaborating with patients, families, physicians, payors, and providers across the continuum of care

  • Conduct face-to- face assessments and document primary and back-up discharge plan on all patients

  • Serves as a resource person for families, patients, employees and other health care professionals, needing information on Mercy Medical Center-North Iowa services and community services(communication)

  • Assists in creating processes to anticipate and address specific situations within the department, including post-acute resource referral lists, denials, readmissions, and Length of Stay

  • Applies quality improvement methods and techniques to improve case management processes to maximize costs and quality benefits for MercyOne North Iowa Medical Center

  • Participates in comprehensive team meetings and conferences regarding specific patient needs that affect cost, quality, and length of stay.

  • Demonstrates ability to promote collaboration and creativity among members of the health care team.

  • Maintains required department confidentiality and abides by HIPPA regulations.

  • Provides competent patient care to the above indicated patient populations by obtaining and appropriately interpreting data, making appropriate decisions based on demonstrated knowledge of age-specific growth and development, and appropriately adapting procedures and treatments.

  • Takes an active role in enhancing ability to carry out job functions through personal and professional/job-related growth and development, and participates in medical center and/or departmental education programs.

  • Communicates with and performs duties following the philosophy of MercyOne North Iowa Medical Center and the Sisters of Mercy reflecting the organization’s Mission, Values, and Guiding Behaviors.

  • Supports and abides by all medical center, departmental, and safety policies and procedures. Proactively identifies safety concerns surrounding the work environment.

  • Performs other responsibilities as requested by supervisor, director or department head.
