Job summary

Trinity Health’s MercyOne Dubuque location seeks a Care Manager RN II to coordinate clinical and discharge planning, ensuring timely progression of care and safe transitions across care settings. The role requires independent practice and collaboration with interprofessional teams in a Magnet-designated system.

Qualifications

• Graduate from an accredited school of nursing required.
• BSN is preferred.
• Current Iowa RN license.
• Proof of completion of Mandatory Reporter – Child and/or Dependent Adult Abuse training within three months of hire.
• Colleagues employed in Illinois facilities are exempt from this requirement under state requirements.

Responsibilities

• Coordinate clinical and discharge planning across the care continuum.
• Lead daily progression-of-care rounds to identify barriers.
• Coordinate discharge planning including education, post-acute placement, home services and follow-up.
• Provide clinical consultation and support to staff and payers as needed.
