Job summary

TriStar Health is seeking a Case Manager RN to coordinate safe, efficient care transitions for patients across our Middle Tennessee network.

You will perform clinical assessments, create individualized care plans, and partner with physicians, nurses, and social services to support optimal outcomes and timely discharge, while educating patients and families and advocating for their needs according to TriStar’s patient-first standards.
