Job summary

St. Davids Healthcare is seeking a dedicated Case Manager (RN) to support patients and families through their care journey.

You will coordinate services, connect resources, and develop individualized care plans in collaboration with physicians, nurses, and leaders to ensure safe transitions and consistent quality of care. Your role includes guiding patients through orientation, coordinating education for families, and documenting psychosocial assessments and discharge planning needs.

Qualifications

• Licensed RN in good standing
• Bachelor's degree required
• Experience in case management or discharge planning preferred

Responsibilities

• Guide patients and families through the care journey and program orientation
• Coordinate education for patients, families, and caregivers to participate in treatment planning
• Develop and implement individualized treatment plans reflecting patient strengths and needs
• Complete psychosocial assessments and discharge planning evaluations with clear documentation
• Collaborate with Rehab Program Director and interdisciplinary team for patient-centered care
• Promote HCA Healthcare values of compassion, respect, and excellence in every interaction
