Job summary

- 2-3 years case management/home health experience
- Associates degree in nursing required; BSN preferred
Responsibilities
- Serve as liaison with members, families, providers, and insurers.
- Coordinate all case management activities for complex cases across care continuum.
- Interact with members in person or by phone; conduct home/office visits as needed.
- Assess medical and vocational status; plan care to optimize wellness and return to work.
- Communicate with stakeholders and document case work thoroughly.
