Job summary

Dameron Hospital is seeking a skilled Case Manager to coordinate and monitor daily operations of Case Management and Utilization Management programs. The role focuses on ensuring appropriate utilization of hospital resources while upholding high standards of patient care.

Candidates should be a registered nurse with a current state license, and have 1+ year of case management experience, leadership abilities, and strong communication and computer skills. CCM certification is preferred.

Qualifications

• Registered Nurse, graduate from an accredited school of nursing.
• Must hold a current state license and renewal per State Board of Nursing.
• Three years acute care clinical nursing experience.
• Minimum of 1-year Case Management experience.
• Additional education in Quality Management and Joint Commission Standards.
• Long term acute care and rehabilitation experience is preferred.
• Demonstration of leadership, manageability, and the application of interpersonal and supervision/administration principles.
• Must be able to read, write, and speak English with good verbal and written communications.
• Good computer skills with minimal guidance.
• Certification in Case Management (CCM) preferred.

Responsibilities

• The Case Manager coordinates and monitors day-to-day operations of the Case Management and Utilization Management programs.
• Directs activities to ensure appropriate utilization of the Hospital and its resources while maintaining optimal patient care standards.
