Qualifications

• VT/compact RN license required.
• AHA BLS certification required.
• BSN degree required.
• Minimum 3 years of clinical experience.
• Recent acute care hospital experience preferred.
• Travel experience and utilization review background are a plus.

Responsibilities

• Coordinate comprehensive care for patients across the continuum.
• Conduct assessments and develop care plans.
• Advocate for patient needs and facilitate transitions between care settings.
