Job summary

Valor Healthcare, Inc. is seeking a Non-Clinical Case Manager to support VTARNG service members, coordinating all aspects of care, ensuring proper utilization of services, and assisting within and outside VTARNG facilities.

The role requires an active RN license (Vermont or ability to obtain), with BSN preferred, and at least 2 years in case management or care coordination. Strong organizational and documentation skills are essential, and U.S. citizenship is required.

Qualifications

• Active, unrestricted Registered Nurse license is required to perform non-clinical case management services.
• BSN preferred but not mandatory.
• Minimum of 2 years in case management, care coordination, or utilization management.
• Strong organizational and documentation skills; ability to work independently in a federal environment.

Responsibilities

• Support Periodic Health Assessments and Soldier Readiness Processing by providing case management and coordinating care activities.
• Maintain electronic case management profiles within required timelines after provider referrals.
• Prepare monthly case status reports tracking soldier care progression and readiness outcomes.
• Ensure compliance with privacy and HIPAA standards in all case management activities.
• Document medical and administrative information accurately across required systems.
