The role requires active nursing licensure in Florida, with CCMC certification preferred. This is a full-time day shift in Case Management at Ascension Sacred Heart Pensacola.

Qualifications

• Licensed RN in Florida required or pending licensure at hire.
• CCMC certification preferred (ACM also accepted).

Responsibilities

• Coordinate interdisciplinary care plans from admission to discharge.
• Monitor medical necessity and level of care via assessments and records.
• Maintain ongoing communication with care teams to implement and reevaluate plans.
• Serve as liaison for post-discharge services and medical record documentation.
