Job summary

Ascension Sacred Heart Bay in Panama City, FL is seeking a Registered Nurse to join the Case Management team on a day shift. You will develop and oversee care plans across the continuum, aligning with patient and family preferences and ensuring quality post-discharge arrangements.

Qualifications

• Licensed as a Registered Nurse in Florida required prior to hire.
• BLS Provider preferred (American Heart Association or Red Cross).

Responsibilities

• Create care plans across the continuum aligning with patient/family preferences.
• Monitor patient care through assessments and records.
• Advocate for resources and removal of barriers.
• Maintain ongoing dialogue with supervisor and health providers to implement the health plan.
• Ensure medical record documentation supports diagnoses and post-discharge care.
