Job summary

HCA Houston Healthcare Southeast in Pasadena, Texas seeks an RN Case Manager to guide patients and families through care transitions, coordinate education, and develop personalized care plans. The role collaborates with physicians, nurses, and leaders to ensure safe transitions and high-quality, evidence-based care across services.

The ideal candidate holds an RN license in Texas with 2+ years in case management or 3+ years in clinical nursing, and a BSN is preferred.

Qualifications

• RN license in Texas (or TX Compact)
• 2+ years in case management OR 3+ years in clinical nursing
• Bachelor’s degree in Nursing preferred
• Graduate of accredited nursing program

Responsibilities

• Guide patients and families through orientation, discharge planning, and benefits explanations
• Coordinate education for patients, families, and caregivers for treatment planning
• Develop individualized treatment plans reflecting patient needs and goals
• Complete psychosocial assessments and communicate needs to the care team
• Collaborate with Rehab Program Director and interdisciplinary team for seamless care
• Promote HCA Healthcare values in every patient and family encounter
