Job summary

Protouch Staffing seeks an experienced Registered Nurse Case Manager in California to support patients and families with high-level assessment, care planning, and transitions. The role emphasizes interdisciplinary collaboration, adherence to utilization criteria, and advocating for patient choice, with emphasis on cultural competence and accurate documentation.

Shifts are 8am–5pm, 40 hours weekly. Must hold a current CA RN license and BLS; CCM/ACM case management credentials are preferred.

Qualifications

• Minimum five years of clinical experience with at least one year in Case Management.
• Proficiency in acuity levels for patient populations and care planning.
• Strong customer service skills and English communication proficiency.
• Ability to manage multiple concurrent projects and coordinate care.
• Familiarity with Joint Commission, CMS, CDPH guidelines; CCS knowledge is a plus.
• Bilingual abilities are preferred.

Responsibilities

• Lead high-level assessment and reassessment to set care priorities.
• Plan, organize, and implement goal-directed patient care and transitions.
• Collaborate with interdisciplinary teams to ensure quality, safe care.
• Advocate for patient self-determination and cultural competence.
• Utilize InterQual criteria for appropriate level of care and LOS.
• Ensure accurate documentation and liaison with medical staff and payers.
