Luminis Health, Inc. is seeking a Case Manager to coordinate patient care across the continuum and support safe, timely transitions.

You will assess needs, plan services, implement care plans, and engage patients using a biopsychosocial approach and evidence-based guidelines. Requirements include Maryland RN licensure, a BSN or ADN with equivalent experience, and at least 3 years of clinical experience in ambulatory or post-acute care.

Formación

• BSN or ADN with equivalent experience; BSN must be achieved within 5 years.
• Three years of experience in a clinical setting, ambulatory or post-acute.
• Current licensure as a registered nurse by the Maryland Board of Nursing.

Responsabilidades

• Identifies and prioritizes patients in need of care management services using a holistic approach.
• Promotes patient engagement and self-care through motivational interviewing.
• Uses evidence-based guidelines to promote quality care and reduce unnecessary days; documents actions in Epic.
• Manages observation stay patients and coordinates transitions to appropriate care settings.
• Maintains clear, concise documentation reflecting patient needs, plans, and referrals.
• Participates in care management process improvements and cross-functional initiatives.
• Applies risk tools to mitigate barriers and prevent readmission.
• Ensures compliance with CMS and commercial insurer requirements.
