Cook Children’s Health Care System in Fort Worth, TX is seeking an experienced RN Case Manager to coordinate the longitudinal care of patients with complex medical and social needs.

You will act as the primary liaison between patients, medical providers, and the insurance/benefit system, ensuring optimal outcomes and efficient resource use. This role offers 8:00 am–4:30 pm shifts with weekend coverage as needed, and PRN staff fill-ins throughout the week.

Qualifications

• Experience in coordinating care for patients with complex medical and social needs.
• Strong communication and advocacy skills for patients, families, and care teams.
• Ability to navigate insurance/benefit systems and care plans.
• Knowledge of clinical pathways and discharge planning.
