Crest-Home-Health-and-Hospice is seeking an RN Case Manager to oversee patient care through visits and coordination with multiple disciplines. You will be the main contact for care discussions, manage visit frequency, and ensure accurate OASIS data collection for each episode.

The role requires Idaho RN or LPN licensure and CPR, with strong communication and critical thinking skills to navigate diverse clinical settings while maintaining professional standards.

Competenze

• Must hold Idaho state RN or LPN license (unencumbered).
• CPR certification required.
• Clinical experience in home health preferred.
• Excellent communication and professional demeanor under stress.

Mansioni

• Perform visits and coordinate patient care across disciplines.
• Oversee frequency of visits for the episode and act as point of contact for care team.
• Consult with physician and office with patient care details.
• Perform accurate OASIS collection and maintain current medication profiles.
• Ensure timely reporting of lab values to the physician and participate in case conferences.
