Job summary

Celebrity Home Health, Inc. is seeking a registered nurse case manager to coordinate skilled home health services, complete assessments and OASIS documentation, and develop and update plans of care to promote safe patient outcomes.

The role involves providing skilled nursing visits, completing timely assessments and care plans, ensuring physician communication, patient education, medication review, and interdisciplinary management while adhering to Medicare, HIPAA, infection-control, and agency

Qualifications

• Active Nevada RN license or valid compact privilege.
• Current BLS/CPR.
• Reliable transportation, driver’s license, and auto insurance for field duties.
• Home health and OASIS experience preferred.
• Employment is contingent on credential verification, background and exclusion screening, competency validation, and required health clearances.
