Crest-Home-Health-and-Hospice is seeking an RN Case Manager to oversee patient care through visits and coordination with multiple disciplines. You will be the main contact for care discussions, manage visit frequency, and ensure accurate OASIS data collection for each episode.

The role requires Idaho RN or LPN licensure and CPR, with strong communication and critical thinking skills to navigate diverse clinical settings while maintaining professional standards.

Qualifications

• Must hold Idaho state RN or LPN license (unencumbered).
• CPR certification required.
• Clinical experience in home health preferred.

Responsibilities

• Perform visits and coordinate patient care across disciplines.

Case ManagerRESPONSIBILITIESIn addition to performing visits and completing coordination of client care, the RN Case Manager is the point of contact for all disciplines involved with providing care to patients and oversees the frequency of visits for the episode. The RN Case Manager consults as needed with the physician and the office giving details about patient care. Performing accurate OASIS collection, ensuring the medication profile remains current, ensuring lab values have been reported to the physician timely, attending weekly case conference and monthly case manager meetings are key responsibilities of this role.Provide all patient care in a manner that reflects respect for patient rights, dignity, and expressed needsAssess the patient’s physical, psychological, and social needsAssess pain and implement appropriate pain management techniquesDocument patient history, symptoms, assessments, tests, and care according to guidelinesReview patient history, diagnostics, and lab data, and report any abnormalitiesQUALIFICATIONSMust have a current, unencumbered Idaho state RN or LPN licenseCertification in CPRPrior clinical experienceExcellent communication skillsDemonstrated ability to maintain a high level of professionalism during stressful timesStrong critical thinking skillsAbility to adapt to different clinical settings and environmentsPay: $45.00 - $55.00 per hourBenefits:* Dental insurance* Health insurance* Vision insurance
