Job summary

Mayo Clinic in Wisconsin (La Crosse) seeks an experienced Registered Nurse Case Manager to coordinate patient plans across the continuum of care. You will work with an interdisciplinary team to ensure utilization management, care coordination, and timely, cost-effective services while maintaining high-quality outcomes.

Successful candidates will hold an RN license, have 3 years acute nursing experience with case management preferred, and may pursue CCM/ACM-RN certification.

Qualifications

• RN license in the applicable state.
• 3 years acute nursing experience preferred; 1 year case management experience preferred.
• CCM or ACM-RN certification preferred.

Responsibilities

• Coordinate patient plans of care across the continuum of care.
• Support utilization management and resource coordination.
• Provide leadership and education on case management concepts to the team.
