Job summary

Great Falls Clinic Hospital is seeking a dedicated Case Manager to coordinate patient care across disciplines, promote cost-effective resource use, and ensure compliance with case management and discharge planning standards.

Responsibilities include collaboration with medical staff, UR committee, and leadership; you will guide utilization review, maintain high nurse-to-patient ratios, and support patient outcomes while delivering compassionate, team-based care.

Qualifications

• RN license (MT state or compact) required.
• Minimum 2 years healthcare experience as an acute care case manager; 3 years preferred.
• Utilization Review, Case Management, and Quality Review/Management experience preferred.

Responsibilities

• Coordinate patient care treatment and services based on patient needs.
• Promote cost effective allocation of resources while maintaining high quality of care.
• Comply with accrediting body standards for Case Management/Discharge Planning and Utilization Review.
• Work with Case Management staff, Hospitalists, UR Committee Members and leadership.
