Job summary

Qualifications

• RN license required; CCM/ACM/CMC preferred.
• Minimum 2 years acute care case management experience; 3 years preferred.
• Utilization Review and case management experience a plus.

Responsibilities

• Coordinate patient care and services based on needs.
• Lead discharge planning to ensure timely and safe transitions.
• Ensure compliance with accrediting standards and UR activities.
• Collaborate with hospitalists, staff, and leadership to optimize care.
