Job summary

Qualifications

• Current Montana RN license.

Responsibilities

• Completes comprehensive assessments and determines eligibility for case management.
• Develops and implements a case management plan with the member, caregiver, physician and other healthcare professionals.
• Performs ongoing monitoring of the care plan to evaluate effectiveness and adjust as needed.
• Maintains ongoing member caseload with regular outreach and management.
• Promotes integration of services including behavioral health care and long-term supports to enhance continuity of care.
• Performs other related duties as assigned.
