• BSN is required.
• Master's degree preferred or in progress.
• Current Michigan RN license is required.
• Minimum 5 years of clinical RN experience.
• Recent case management experience is required.
• At least 1 year of Trinity Health experience preferred.
• Strong communication, critical thinking, and interpersonal skills.
• Knowledge of utilization review, payer systems, and regulatory compliance.
• Proficiency in electronic medical records and MS Office tools.

Responsabilidades

• Coordinates patient care using clinical pathways and interdisciplinary conferences.
• Assesses patients using evidence-based criteria and determines levels of care.
• Monitors resource utilization, outcomes, and patient progress.
• Facilitates communication among healthcare teams, patients, and families.
• Acts as a resource for discharge planning and education.
• Participates in quality improvement and care redesign initiatives.
• Maintains thorough documentation in the electronic health record system.
