Trinity Health is seeking an RN Care Manager in Michigan to join the primary care team. The role involves assessment, care planning, and coordination to prevent readmissions and address medical, environmental, and psychosocial needs across the continuum of care.

The ideal candidate holds a BSN, Michigan RN license, active CPR, and a valid driver's license with at least three years of acute care or case management experience. Flexible scheduling and hospital-to-community coordination are included.

Qualifications

• Graduation from an accredited BSN program.
• Current Michigan RN license, CPR certification, and valid Michigan driver's license.
• Three years of acute care or case management experience.
• Two years of utilization or HMO experience preferred.
• Ability to work in a fast-paced environment with multiple tasks and patients.
• Possible exposure to infectious disease, hazardous chemicals, and blood and body fluids.
• Ability to work flexible schedules.

Responsabilités

• Keeps member/family informed and requests further assistance when needed.
• Demonstrates ability to follow through with requests, critical information, and timely responses.
• Functions as liaison between administration, members, physicians and other healthcare providers.
• Interacts professionally with member/family/physicians in the plan of care formation.
• Performs Health Risk Assessment (HRA) and determines acuity score for follow-up.
• Develops and implements an outcome-based plan of care and documents it in the member's medical record.
• Maintains NCQA standards alignment and manages patient caseload to meet contract goals.
