Barton Health in California is seeking an RN Case Manager to coordinate patient care across the continuum, focusing on timely discharge planning and utilization review.

You will work with patients, families, physicians and the care team to ensure appropriate levels of care and seamless transitions, while maintaining compliance with Medicare/Medicaid and payer requirements.

Requires CA RN license, BLS, 3 years’ acute hospital case management; bachelor’s degree preferred.

Formación

• Completion of an accredited Nursing program.
• Bachelor's degree preferred.
• 3 years’ experience in an acute hospital setting
• In compliance with patient safety standards, must be able to effectively communicate in English; Bilingual abilities preferred.

Responsabilidades

• Provides case management assessments within organizational timeframes.
• Develops discharge plans in collaboration with the multidisciplinary team.
• Performs utilization reviews using InterQual criteria.
• Coordinates timely discharges and transfers to appropriate levels of care.
• Maintains knowledge of Medicare/Medicaid and payer requirements.
