HCA HealthONE Swedish in Colorado is seeking an RN Case Manager to coordinate services through an interdisciplinary process, ensuring appropriate admission, hospitalization, and discharge planning. The role requires active Colorado RN licensure and 3–5 years of case management experience.

BSN is preferred and strong communication as well as critical thinking skills are essential to collaborate with the care team and manage care efficiently.

Qualifications

• RN licensure in Colorado required or multi-state licensure active.
• BSN preferred.
• 3–5 years of recent clinical or case management experience.

Responsabilités

• Coordinate services through an interdisciplinary process across the continuum of care.
• Assess admission appropriateness, continued hospitalization, and level of care.
• Develop and reassess discharge planning from admission onward.
• Monitor quality and risk management and report to the CQI process.
