Job summary

HCA HealthONE Swedish is seeking a Case Management Coordinator to oversee the coordination of services across the continuum of care. The role requires leading interdisciplinary teams to ensure appropriate admission, utilization of resources, and effective discharge planning from the hospital setting.

Responsibilities include collaborating with directors and staff, mentoring new team members, and ensuring compliance with federal and state regulations.

Qualifications

• Knowledge of UM/CM, regulatory requirements, and payer processes.
• Experience in hospital UM/CM preferred.
• Leadership and management experience in UM/CM settings.

Responsibilities

• Coordinate services through an interdisciplinary process for clinical, financial, and psychosocial care.
• Act as primary contact for discharge planners and social workers.
• Monitor length of stay issues and barriers to discharge and report to administration.
• Develop staff orientation plans and ongoing competency evaluations.
