HCA HealthONE Aurora is seeking an RN Case Manager (PRN) to support patients and families through their care journey in Aurora, CO. You will coordinate services, develop individualized care plans, and collaborate with physicians, nurses, and leaders to ensure safe transitions and clear communication.

The role emphasizes patient-centered care, discharge planning, and multidisciplinary teamwork within a large national healthcare system.

Competenze

RN license required.
2 years CM experience OR 3 years clinical nursing experience required.

Mansioni

Guiding patients and families through program orientation, explaining rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights
Coordinating education for patients, families, and caregivers to encourage participation in treatment planning, goal discussions, and family conferences
Developing and implementing individualized treatment plans that reflect the patient’s strengths, needs, and personal recovery goals
Completing psychosocial assessments and discharge planning evaluations, clearly documenting findings and communicating needs across the care team
Collaborating with Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient-centered care
