Job summary

Hackensack Meridian Health invites a qualified RN to join as a Case Manager in New Jersey. You will assess patients for admission appropriateness, plan and coordinate care across the continuum, and advocate for patients and families.

The role requires nursing experience and knowledge of CM, utilization review, and discharge planning. The position emphasizes collaboration with physicians, insurance coordination, and adherence to HQSI standards, with a comprehensive benefits package and pay

Qualifications

• Graduate of an accredited Nursing Program.
• Related Case Management and Nursing experience.
• Knowledge of CM, Utilization Review, Discharge Planning and Coordination of Care.
• Excellent computer skills

Responsibilities

• Evaluates all patients for appropriateness of admission and level of care within 24-48 hours.
• Initiates discharge planning within 24-48 hours of admission.
• Discusses initial plan of care with MD and with patient/family.
• Ensures CMS protocols and required documentation are followed.
• Maintains ongoing communication with physicians, patients and families about plan of care.
• Participates in daily multidisciplinary rounds and collaborates across continuum of care.
• Documents and communicates issues to appropriate departments and teams.
• Adheres to organizational competencies and standards of behavior.
