Job summary

Hackensack Meridian Health is seeking a Case Manager responsible for assessment, care planning, and coordination of care for patients and their families. You will evaluate admission eligibility, initiate discharge planning, and collaborate with physicians and families to ensure timely, appropriate plans of care.

The role requires RN licensure in New Jersey and prior Case Management experience, with strong computer skills and knowledge of utilization review processes.

Qualifications

• Graduate of an accredited Nursing Program.
• Related Case Management and Nursing experience.
• Knowledge of CM, Utilization Review Discharge Planning and Coordination of Care.
• Excellent computer skills.

Responsibilities

• Evaluates all patients for admission and level of care within 24-48 hours; initiates discharge planning within 24-48 hours of admission.
• Documents and communicates to physicians, patients and families to ensure an appropriate plan of care.
• Ensures CMS protocols, IM letters and HQSI letters are followed and documented.
• Participates in daily multidisciplinary rounds and maintains communication across the continuum of care.
• Manages communications with insurance companies to support payment and streamline denials processes.
• Contributes to PI goals and patient satisfaction initiatives and supports department projects.
