Job summary

Hackensack Meridian Health is seeking an experienced RN Case Manager Patient Doctor (PD) to coordinate patient care across the continuum, ensure evidence-based practice standards, and manage discharge planning. This role integrates utilization management, care coordination, and discharge planning within units such as ETD, Observation, and PACU, with shift and weekend rotation as needed.

Qualifications

• Case Manager, Registered Nurse background; NLN/AACN nursing program preferred.
• Excellent interpersonal communication and negotiation skills.
• Current knowledge of discharge planning, utilization management, case management, and managed care reimbursement.

Responsibilities

• Coordinate patient care progression across the continuum with the multidisciplinary team.
• Apply InterQual guidelines to monitor admission appropriateness and LOS.
• Manage discharge planning with patient/family, physician, and external stakeholders.
• Communicate with financial counselors and payers to facilitate reimbursement and coordination of care.
• Support performance improvement initiatives and ensure regulatory compliance.
