Vighter is seeking a Registered Nurse (RN) Case Manager to provide patient care and nursing administrative support for outpatient services at Joint Base Andrews, MD, in accordance with policies and guidelines while supporting Service Members, Veterans and families.

The role requires 3 years (or 2 with BSN) of nursing and care coordination, an active RN license, and BLS/ACLS. Travel may be required; comprehensive benefits accompany a competitive hourly rate.

Qualifications

• Minimum 3 years (AND) or 2 years (BSN) of nursing and care coordination.
• Active RN license in any US state.
• Current BLS and ACLS certifications.
• CCM or ANCC certification (or eligible).

Responsabilités

• Assess patients, develop care plans, coordinate services.
• Coordinate case management across inpatient, outpatient, telehealth and community settings.
• Maintain patient care documentation and records.
• Collaborate with interdisciplinary teams and community resources.
• Support discharge planning and follow-up care.
• Travel to off-site sites as required.
