Qualifications

• Bachelor's degree in Nursing required or strongly preferred in market regions.
• Current RN license in state of practice or multi-state licensure (NLC) where applicable.
• BLS certification within 60 days of hire.
• CCM certification or equivalent specialty certification encouraged when eligible.

Responsibilities

• Provide individualized nursing care using the nursing process from assessment to evaluation.
• Coordinate care with licensed practitioners and inter-professional team members.
• Assist discharge planning and referrals to post-acute resources; educate patient and family.
• Advocate for patients across care settings to ensure desired outcomes and appropriate level of care.
