Sanford Health’s Marsh Med Ctr in Marshfield, WI is seeking a part-time Case Manager with RN licensure to coordinate patient care across the hospital stay and after discharge. You will collaborate with physicians, nurses, and multidisciplinary teams to optimize outcomes and resource use.

Responsibilities include discharge planning, facilitating post-acute referrals, and ensuring quality, cost-effective care for defined patient populations.

Qualifications

• Bachelor's degree in Nursing required/strongly preferred.
• Unencumbered RN license in the practicing state.
• BLS certification within 60 days of hire.
• CCM certification encouraged when eligible.

Responsabilités

• Provide leadership in care coordination across the care continuum.
• Collaborate with licensed practitioners and inter-professional teams.
• Coordinate discharge planning and post-acute referrals.
