Job Summary

The RN Case Manager I is a registered nurse responsible for coordinating the care and services for an assigned unit-based patient population. The RN Case Manager I promotes quality of care, supports effective resource utilization, and facilitates the patient's progression of care to proactively plan for the patient's safe and timely discharge to the most appropriate level of care. The RN Case Manager I serves as the key resource for the patient's multidisciplinary care team and works collaboratively with all members of the patient's health care team and patient representatives to achieve desired clinical, financial and quality outcomes. The RN Case Manager I analyzes current systems and variances to prevent delays and identify opportunities for improvement. This position reports to the Manager, Case Manager or Director. Case Management is a collaborative process that assesses, collaborates, plans, implements, coordinates, monitors, and evaluates options and services to meet an individual's health needs through communications and available resources to promote quality, cost-effective outcomes. All RN Case Manager I activities will be patient centered, proactive, require ongoing assessment of needs, evaluation of resources, and education as needed. The RN Case Manager I is responsible for presenting options in an unbiased manner and meeting compliance with HIPAA. Advocacy and education are at the heart of all activities focusing on providing the best possible clinical outcomes. Case Management activates its processes to secure the right service to the right patient at the right time at the right level of care.

Schedule

Mon - Fri 8:30a - 5p. The scheduled start and end times require flexibility to meet the needs of the practice.

  • 4 year Bachelor's Degree - OR - combination of relevant education & experience may be considered in lieu of Bachelor's Degree
