Qualifications

• Experience making referrals to home health, infusion, or DME.

Responsibilities

• Coordinate care and facilitate access to tests, procedures, and results for discharge readiness.
• Assist in discharge planning and continuity of care across departments.
• Serve as liaison to ensure smooth transitions and advocate for patient needs.
• Provide case-management training and educate staff on processes.
• Collaborate with SW-Case Manager and refer high-risk patients as needed.
